Home of Hope provides community support services tailored to individuals with disabilities, enabling them to live independently or with their families. These services are customized based on each person's abilities and needs, ranging from a few hours of support to round-the-clock care.
The organization's staff members are trained to assist individuals with various levels of developmental disabilities, including those with dual diagnoses or who are medically fragile.
Through our program, individuals are guided in developing essential life skills that promote greater independence, including medication management, meal preparation, personal laundry, money management, hygiene, and community integration. We aim to help them build confidence in their daily routines while ensuring their safety and well-being. Residents will be supported by a dedicated team of professionals based on individual need. These may include speech and physical therapists, registered or licensed practical nurses, a dietician, pharmacist, psychiatrist, case manager, and trained Direct Care Support Professionals—all working together to provide the highest level of care.
Where legal guardians are involved, they partner with the resident and our team. Personalized Individual Habilitation Plans are created that are tailored to individual needs and abilities. This ensures that every step taken is one toward greater independence and, when possible, a transition into a less restrictive environment.
